Registration Information

Parents of all students will need to complete registration and emergency contact forms.  Copies of necessary forms are below for parents to print and complete prior to registration.

New students will need at least 2 proofs of residency.  Proof of residency may include a current utility bill, occupancy permit, current real estate tax bill, current homeowner or renter insurance bill, homeownership title or deed, lease, paycheck stub with an address in the district, and driver's license with an address in the district. Parents of students transferring from another school will need to complete transfer papers and other necessary paperwork. Students entering kindergarten or preschool will also need an original birth certificate and a copy of his/her social security card.

 

Requirements for starting school

Kindergarten Requirements:   Students entering kindergarten for the first time are required to have a school physical examination and updated immunizations. Those two items are due by September 15th, or 30 days after your child’s first day of school. Failure to turn in these items will result in your child being excluded from school until the proper forms are received.

 

Students will need to have the following immunizations on record:

DPT- 4, with the last one being after their fourth birthday

Polio- 3, with the last one being after their fourth birthday

Measles- 2, first dose after their first birthday and second dose no less than 4 weeks after the first

Mumps- 2, first dose after their first birthday and second dose no less than 4 weeks after the first

Rubella- 2, first dose after their first birthday and second dose no less than 4 weeks after the first

Varicella- 2, first dose after their first birthday and second dose no less than four weeks after the first or date of disease recorded

 

Kindergarten students will also need a dental exam and an eye exam. The eye exam is due by September 15, 2018, and needs to be completed by an eye doctor. The dental exam is due by May 15, 2019. The Mobile Dentists (Dental Safari) are scheduled to visit Marissa schools in November. Please watch for information about Dental Safari to go home in September. If you want to fill out waivers for either the eye or dental exam please see the nurse.

 

Second Grade Requirements:  All second-grade students are required to have a dental exam during their 2nd-grade year. The exam is due by May 15. The Mobile Dentists (Dental Safari) are scheduled to visit Marissa schools in November. Please watch for information about Dental Safari to go home in September. If you would like a dental waiver please see the nurse.

 

Sixth Grade Requirements:  All sixth-grade students are required to have a school physical examination and updated immunizations. These items are due by September 15, or 30 days after your child’s first day of school. Failure to turn in the required forms will result in your child being excluded from school until the proper forms are received. Students entering the sixth grade are also required to have a dental exam by May 15th, 2019. The Mobile Dentists (Dental Safari) are scheduled to visit Marissa schools in November. Please watch for information about Dental Safari to go home in September. If you would like a dental waiver please see the nurse.

 

Ninth Grade Requirements:  All ninth-grade students will be required to have a school physical examination and updated immunizations. These items are due by September 15, or 30 days after your child’s first day of school. Failure to turn in the required forms will result in your child being excluded from school until the proper forms are received.

 

Twelfth Grade Requirements:  All twelfth-grade students will be required to show proof of updated immunizations. The state of Illinois requires that twelfth graders shall show proof of having received two doses of meningococcal conjugate vaccine prior to entering the 12th grade.  The first dose shall have been received on or after the 11th birthday, and the second dose shall have been received on or after the 16th birthday, at least eight weeks after the first dose.  If the first dose is administered when the child is 16 years of age or older, only one dose is required.

 

New Students:  Students new to Marissa School District are required to have a school physical and up-to-date immunizations on file. These items will need to be turned in 30 days after your child’s first day of school. If your child is in kindergarten, 2nd, 6th, 9th or 12th grade, please see those requirements as well. Failure to turn in the required forms will result in your child being excluded from school until the proper forms are received.

 

New Students from Out of State:   All new students from out of state are required to have an up-to-date Illinois school physical on file. All immunizations must be current and a current eye exam performed by an eye doctor is also required. If your child is in kindergarten, 2nd, 6th, 9th or 12th grade, please see those requirements as well. Failure to provide the required school physical and immunization record 30 days after your child’s first day of school will result in your child being excluded from school until the proper forms are received.
